Attention parents and guardians of Valley 6th grade students:
A free dental program will be at Valley Middle School. This program helps to prevent tooth decay. A dental hygienist will screen your child's teeth and decide which teeth need to be sealed. A dental hygienist will then put the sealants on your child's teeth to seal out food and bacteria that cause decay. Your child's sealants will be checked next year. New sealants will then be applied if needed. Please fill out the purple flyer that was sent home with your child.
Please return this form to the VMS Nurse as soon as possible. Forms are due September 22, 2023.
